If you are applying for a License by Examination, or have previously submitted an application and need to pay your fee for a temporary license or criminal history background check, complete the information below.
All applicants must submit verification of a valid fingerprint clearance card from the Arizona Department of Public Safety (“DPS”), or submit a full set of fingerprints on an FD-258 card and a $40 fee so the Board can request a criminal history background check from DPS.
